(Invisalign + Veneer)

Terms & Conditions

1. Purpose of the Membership

The AuraDent Premium Membership is designed to:

• Maintain Invisalign treatment results long-term
• Protect veneer restorations from damage and wear
• Provide continuous monitoring of tooth position, bite, and stability
• Support patients combining orthodontic and cosmetic treatments

This membership is built for patients who require advanced, long-term protection of both alignment and restorations.

All benefits are based on continuous clinical follow-up and compliance.

2. Included Services

As an active Premium Member, you receive:

A. Invisalign Maintenance

• 2 clinical check-ups per year (including digital scans and evaluation)
• 1 set of Vivera retainers every 6 months (total 2 sets per year)
• Tooth movement tracking and long-term stability monitoring

B. Veneer Protection

• Veneer condition monitoring (including bite and pressure analysis)
• 1 custom-made night guard per year

C. Premium Benefits

• 1 professional at-home whitening kit per year
• 30% discount on general dentistry treatments
• Priority booking (appointments available within 24 hours when possible)
• Access to exclusive AuraDent products

D. Additional Clinical Support

• Bonded retainer repair or replacement if damaged or loosened
• Access to discounted emergency retainers (Essix retainer: 1,900 SEK)

3. Mandatory Follow-Up Requirement

To maintain membership and eligibility:

• The patient must attend check-ups every 6 months

A grace period of 45 days is allowed.

If the patient does not attend within:
6 months + 45 days

Then:

• Membership is automatically canceled
• All benefits are terminated
• All guarantees become invalid

4. Appliance & Protection Policy

Invisalign (Retainers)

• Vivera retainers are delivered after completed check-ups
• Lost retainers are not replaced free of charge
• Temporary retainers may be provided at discounted cost

Veneers (Night Guard)

• Night guard must be used as instructed to protect veneers
• Lost or damaged night guard may require replacement at additional cost

Bonded Retainers

• Repaired or replaced if damaged or loosened at no additional cost
• Patient must contact AuraDent immediately

If delayed:

• Tooth movement or veneer stress may occur
• Case will be evaluated and treated accordingly

5. 5-Year Stability & Protection Guarantee

Applies to both:

Invisalign results
Veneer restorations

Guarantee applies only if:

• Treatment was completed at AuraDent
• Membership remains active
• Follow-up visits are completed
• Retainers and night guard are used as instructed

Guarantee includes:

• Clinical evaluation of relapse or damage
• Corrective support based on clinical assessment

Guarantee does NOT apply if:

• Follow-up requirements are not met
• Appliances are not used as instructed
• Damage occurs due to:
• • Trauma or accidents
  • External dental treatments
  • Misuse or neglect

6. Membership for External Patients

Patients treated outside AuraDent:

• Must undergo clinical evaluation
• May be subject to onboarding fee (1,900 SEK)
• Are not eligible for the guarantee

7. ATB (Allmänt tandvårdsbidrag)

For patients eligible through Försäkringskassan:

• Available ATB will be used for preventive services

If unavailable:
No additional cost applies

8. Payment Terms

Option 1 — Monthly Payment

• Fixed monthly fee
• Membership remains active while payments are completed

Option 2 — Upfront Payment

• 12 months prepaid
• 15% discount applied

Payment Commitment

• Membership is structured as a 12-month care cycle

If the patient has used any of the following:

• Check-ups
• Retainers or night guard
• Whitening kit
• Any included service

The full yearly cost becomes binding

9. Cancellation Policy

Patients may cancel at any time.

✔ Cancellation BEFORE using services

• No check-up attended
• No retainers/night guard delivered
• No whitening kit provided

No further obligation

⚠️ Cancellation AFTER using services

If the patient has received or used:

• Check-up
• Retainers
• Night guard
• Whitening kit

Full 12-month payment is required

ATB in Cancellation

• Used ATB is not refundable

10. Membership Validity

Membership remains valid only if:

• Follow-up visits are completed
• Clinical compliance is maintained
• Payments are fulfilled

AuraDent reserves the right to terminate membership if requirements are not met.

11. Transparency Statement

AuraDent Premium Membership is built on:

• Clinical responsibility
• Long-term treatment stability
• Preventive protection of both alignment and restorations

We do not provide guarantees without proper monitoring and compliance.
