How do cavities form and why repair is needed?
Cavities form when acids produced by bacteria in dental plaque break down the enamel. These bacteria feed on sugars and carbohydrates, and as the acids penetrate deeper into the tooth, they reach the softer dentin where decay spreads faster. If left untreated, the decay can reach the nerve, which is why a filling is needed once the enamel is breached. Dentists detect cavities using instruments and X-rays, which help reveal hidden decay and show how close it is to the nerve.
How is a damaged tooth repaired with a filling?
To fill a tooth, all decay is removed using a dental drill. Discomfort usually comes from the vibration and cold water, which prevents overheating. If you feel sensitivity despite anesthesia, ask your dentist for more. The tooth is then prepared for a composite filling—matched to your natural tooth color. The surface is treated with a mild acid and bonding agents before the filling is applied in thin layers and hardened with a special light. The dentist then adjusts the shape so the bite feels natural.

What should you keep in mind after a dental filling?
• After a filling, you can usually eat and drink as normal right away. However, if you’ve had anesthesia, we recommend waiting until it wears off to avoid accidentally biting your cheek or tongue.
• It’s normal for the tooth and surrounding gum to feel a bit tender for a few days. If the discomfort doesn’t improve within a week, contact your dentist for advice.
• Sensitivity to cold is common after a filling and can last for 3–4 months. This happens because the tooth needs time to recover from the decay and the treatment. If the sensitivity gets worse over time, you should reach out to your dentist.
• Remember that a filled tooth isn’t as strong as a healthy, untouched tooth. Avoid biting into hard foods or using the filled tooth to tear or pull.

When a Cavity Is Very Deep
If the decay has reached close to the nerve, your dentist may recommend treating the tooth in two steps to reduce the risk of nerve damage. In these cases, a small amount of decay near the nerve may be left, and a tissue-healing material is placed. The tooth is then temporarily filled to allow time for recovery. After about four months, the tooth is re-examined with X-rays and special tools. If healing has gone well, the tooth can be permanently restored with a composite filling.

If the Nerve Doesn’t Heal Properly
Sometimes the tooth does not respond well to the initial treatment. You may experience sensitivity, aching, or discomfort after a few weeks. These can be signs that the nerve is infected or beginning to die. If this happens, it is important to contact your dentist as soon as possible. To save the tooth and avoid future problems, a root canal treatment may be needed to remove the affected nerve and restore the tooth fully.
